"Cut."

You freeze. Your hand hovers over the skin. The blade is still. The air is thick. It smells of iron and sweat. You are the Surgeon. Or so they call you. You hold the knife. You hold the truth.

"Again," says the Magistrate. He sits in the shadow. His face is a mask of bone. "Do it properly this time, Doctor."

You nod. You do not speak. You cannot. Your throat is closed. You are a prisoner of your own skill. You have been here for three days. The city outside is screaming. The plague is not just a sickness. It is a voice. It whispers through the walls. It eats the minds before it eats the bodies.

You look at the patient. He is young. A boy. No older than sixteen. His eyes are open. They are bright. They are terrified.

"Who are you?" you ask. Your voice is a rasp.

"I am Thomas," he says. "I am the key."

You laugh. It is a dry, ugly sound. "There are no keys, boy. Only locks. And only rust."

"You know," he says. "You know what is coming."

You lower the blade. You look at the Magistrate. The man is watching. He is waiting. He wants to see you cut. He wants to see the blood. He wants to see the secret spill out like water.

"Do it," the Magistrate commands. "Extract the node."

You move your hand. You approach the neck. The pulse is strong. It beats against your fingertips. It is a drum. It is a warning.

You remember the books. You remember the old texts. The ones they burned. The ones they tried to erase. You know the history. The city is not dying. It is waking up. The plague is the shedding of the old skin. The pain is the birth of the new.

You have known this for years. You have hidden it. You have been a sycophant. A servant. You have served the Magistrate. You have cut out the tumors. You have removed the dissent. You have kept the city safe. Or so you thought.

But the whispers are growing louder. They are in your head now. They are in the boy’s head. They are in the air.

"Cut," the Magistrate says. His voice is cold. It is the sound of a door closing.

You raise the knife. You bring it down.

The blade parts the skin. Blood wells up. It is red. It is hot. It is alive.

The boy screams. It is a sound of pure anguish. It is a sound of pure joy. You do not know the difference. They are the same.

You pull.

You look at the boy. He is calm. The terror is gone. His eyes are clear. He is smiling.

"Thank you," he says.

You feel a surge of power. It is not yours. It is theirs. It is the collective. It is the community. You are part of it now. You are no longer the prisoner. You are the gatekeeper.

But the Magistrate is not done. He draws his sword. It is a thin, bright thing. It is a symbol of his authority.

"Kill him," he says to his guards.

The guards step forward. They are armed. They are ready.

You look at the glowing core in your hand. You look at the boy. You look at the Magistrate.

You make a choice.

You throw the core.

It hits the floor. It shatters.
